A lot of adults live in the five-to-six-hour range and tell themselves the damage is vague, distant, or reversible on the weekend. A new [2026 pooled analysis in _Annals of Internal Medicine_ by Faris M. Zuraikat and colleagues](https://www.acpjournals.org/doi/10.7326/ANNALS-25-01660) suggests the cost may show up faster than that. Across 95 adults at elevated cardiometabolic risk, trimming sleep by about 78 minutes a night for six weeks was enough to produce a modest but measurable increase in body weight, waist circumference, leptin, and sedentary time.

For the analyst’s view, the important point is not the headline number alone. A gain of 0.45 kg over six weeks is not dramatic in isolation, but the design matters. Participants served as their own controls in randomized crossover trials, moving between adequate sleep and restricted sleep with a washout in between. That does more than the usual observational sleep story, which often blurs sleep with stress, shift work, diet quality, or illness. In other words, this paper pushes the field a step closer to saying short sleep is not just associated with weight gain. Under realistic conditions, it can help drive it.

But the skeptic’s objection deserves space early. Six weeks is still six weeks. The sample was small. The participants were already at elevated cardiometabolic risk, and the effect sizes were modest. A reader should not walk away thinking one bad workweek will automatically add fat mass, or that sleep explains most obesity. What the study does show is narrower and more useful: chronic, mild restriction of the kind many adults normalize can shift energy balance in the wrong direction.

That last part is why the paper lands beyond the sleep lab. A [2024 CDC data brief](https://www.cdc.gov/nchs/products/databriefs/db559.htm) estimated that 30.5% of US adults slept less than seven hours a night. From the user-affected perspective, this is not an edge case. It is ordinary life for parents, shift workers, caregivers, and people who keep borrowing from tomorrow’s sleep to finish today’s work. ## What the Annals trial actually showed

In [Columbia’s summary of the trial](https://www.cuimc.columbia.edu/news/skimping-sleep-leads-weight-gain), Zuraikat and colleagues were explicit that the protocol was meant to resemble the way many adults actually live, not an extreme deprivation experiment. Adults with habitual sleep of seven hours or more were asked to sustain adequate sleep during one phase and cut back by 1.5 hours a night during another. In practice, the difference averaged 78.4 minutes. Weight rose, waist circumference rose, whole-body volume rose, and sedentary time climbed by 17.2 minutes a day under restricted sleep.

That matters because much of the earlier literature used sharper cuts over shorter windows. A [2013 randomized study in _Chest_ by Andrew D. Calvin and colleagues](https://doi.org/10.1378/chest.12-2829) found that restricting sleep increased caloric intake without meaningfully changing activity energy expenditure or classic hunger hormones. A separate [2013 study in _Sleep_ by Andrea M. Spaeth and colleagues](https://doi.org/10.5665/sleep.2792) linked short sleep to weight gain, more calories, and later meal timing. Read together, those trials made the causal story plausible. The new Annals paper makes it feel more lived-in. Less total deprivation, more time, older participants, and a result that still moved in the same direction.

For the analyst, that is the strongest contribution. Observational research has long tied short sleep to obesity, but critics could always argue that poor sleep was simply riding along with other risks. Randomized crossover work does not eliminate every confounder, yet it narrows the gap between association and causation. If nearly eighty fewer minutes a night can move weight upward inside six weeks, sleep starts to look less like a background habit and more like a metabolic input.

## Why the mechanism is less tidy than the headline

Here the skeptic gets a second bite. The clean popular explanation for sleep loss and weight gain is hormonal: sleep drops, leptin falls, hunger rises, people eat more. The Annals result was messier than that. Leptin actually increased during restricted sleep. Sedentary time rose too. So did waist circumference.

Useful, because messy mechanisms are usually more believable than one-note ones. A [2022 review in _Nature Reviews Endocrinology_](https://www.nature.com/articles/s41574-022-00747-7) argues that insufficient sleep and circadian misalignment can influence obesity through overlapping pathways: reward-seeking behavior, food timing, glucose regulation, fatigue, and reduced activity, not simply hunger alone. The new trial fits that frame better than the stripped-down story social media tends to prefer.

[Columbia’s summary of the work](https://www.cuimc.columbia.edu/news/skimping-sleep-leads-weight-gain) also carried the clearest public-health translation from the senior author.

There is still room for caution. The study did not run long enough to prove changes in body composition, and it was not powered to settle whether men and postmenopausal women respond differently. Nor does the trial tell readers what happens when lost sleep is repaid on weekends, a question that has become more culturally relevant as “sleep debt” enters mainstream health language. Still, the paper’s own limits are not a reason to dismiss it. They are a reason to keep the claim proportionate.

From the user-affected angle, proportion is the whole point. Someone routinely sleeping five to six hours a night does not need to hear that sleep is “important.” That message has been ambient for years. What this study offers instead is a more concrete translation: mild, sustained restriction can nudge body weight and behavior in an unhealthy direction even before a person feels obviously unwell.

## How far readers can take it

Not all randomized findings deserve everyday extrapolation. This one deserves some, though not all. The participants were adults already at elevated cardiometabolic risk, which means the results are most directly relevant to people who are not metabolically carefree to begin with. Midlife adults, people with central adiposity, and people already trying to manage blood sugar or blood pressure are the obvious readers for whom the trial feels most immediate.

Caution, though, against turning this into a moral tale about personal discipline. Short sleep is often structural. Heat, shift schedules, caregiving, work insecurity, and late-screen work all crowd the same hours. The Annals trial does not solve those upstream problems. It simply makes the metabolic cost easier to defend with data.

Investigators also did not hand readers a weight-loss protocol, and that is a virtue rather than a gap. The study is strongest when read as a reframing paper. In metabolic-health discussions, sleep should sit closer to diet quality, activity, and medication adherence than to generic self-care advice. That does not mean an extra hour in bed is a substitute for other interventions. It means the common habit of treating sleep as negotiable looks less evidence-based than it did a week ago.

So, can losing an hour of sleep a night really cause weight gain? This paper does not prove that every short sleeper will add body fat on schedule. It does something subtler and probably more important. It shows that under randomized, real-world-ish conditions, chronic mild sleep loss is enough to move weight and behavior in a direction nobody managing cardiometabolic risk should shrug off.
